OOC: Just providing a few medical record notes here:
- He’d suffered a deep wound to the upper abdomen that had been sutured.
- Hypovolemic shock due to blood loss
- Given two units of Betazoid type Z blood along with plasma during surgery
- Kayolane 175 ml for sedation and pain had no effect
- Drechtal Beams were used to cut nerve signals and prevent pain sensation
- Blood worked showed echtallin poisoning. It was made to cause nerve damage and hyperactivity, fever but no infection… the body created a crusty residue at the wound from the immune system the nerves, it increased pain, prevented loss of consciousness. It interacted with typical medications, making symptoms worse rather than relieve them. The poison itself was like nervous system acid, damaging and burning the nerves, causing central nervous system dysfunction, which explained his body’s temperature fluctuations. There was the potential for long term nerve damage and organ damage both from the poison itself and the effects of the central nervous system dysfunction. It would cause pain too. It was intended to be a slow, agonizing death as the poison slowly dissolved the nerves and the central nervous system slowly shut down.
Treatment for Echtallin Poisoning:
- hydrolyze treatment - It was a compound that would neutralize the poison, but do it too quickly and it would react with the poison instead and kill him. It had to be given in multiple doses per day for several days.
- Pain medication that blocked pain signals should work, but it had to block the pain signals, because the pain was directly from the nerves. Side effects of the treatment were liver or kidney toxicity, heart problems, numbness, and a whole list of other things. The dosing information was there, describing how to decide how much to give, when, and how.
- and only a specific type of sedative would be able to put him to sleep, because the way it acted to put him to sleep was different than most others.
- Even with the antidote treatment - survival was only 20%
- Given Bromocriptine to help regulate body temperature and hormone functions.

Sisnil nodded. He squeezed her hand, but it was so weak there wasn’t much of a difference. He remained stable, but he was obviously in a lot of pain. The antidote was working though, and after some time, he was able to sit up a little and do small things for himself.
That was when it became more apparent something was wrong. Even when Dee had brought a PaDD, he kept having her do everything with it. He had bumped his hand into a cup of water and knocked it over. Little things like that.

The bumped cup didn’t phase her. Sisnil was weak and his muscles damaged. It was the lack of infuriating independence that concerned her. Even when he was in more pain and delirious from it, Sisnil had insisted on having a PaDD to try and forge his records.
Following her instincts she began to run several scans. “How are you feeling?”

“I’m not worse than I was. I’m fine, Dee,” Sisnil said. “Why?” Her scans showed the poison wasn’t damaging his heart, everything looked about the same. Except… there was damage in the occipital lobe of his brain and damage to the optic nerves.

“Well, you were singularly focused on getting a PaDD and being allowed to work. Now, you have barely looked at it and haven’t asked for any files or to talk to Morris.” She patted his hand, “I know you’re exhausted, I just want to make sure that’s all it is.”
Readying over the results she understood. She sent a quiet message from the console to Tessan along with the scan results.

“You found something,” Sisnil said. It wasn’t a question, it was a statement. He could sense it in her emotions.

“Damage from the Echtallin,” she began to explain when Tessan approached.
Tealor walked in PaDD in hand. He looked at Zoi’s readings a second time. “I see you are awake, tired still?” he asked. “So couple things, you need to not use your gifts , you will heal faster. ” he did not wait for him to respond. “The poison has done some damage to the optic never in your eye, Dradvia lets get a deep scan.”

Chief Medical Officer U.S.S Wolverine“Yes Sir.” Dradvia stepped away long enough to grab the neural scanner. She lowered the bed and then began the scan.

“I’m fine,” Sisnil said, but he always said that. He was going to say something, but Tessan didn’t give him the chance. The news didn’t seem to surprise him. He’d been hiding the vision loss.
The scan showed extensive damage to the optic nerves. The poison had done what it did, sat on and essentially dissolved them. It was throughout his brain, but the worst damage was in the occipital region.

Teasan looked at Zoi, he had been maintaining a barrier around his mind for some time now, he had to have been feeling this since he woke. There was no point in hiding any of it. “Forgive my bedside manner, but you know what we found is bad. We have to find a way to stop the decay in the brain or he’s going to lose more than his eyes. ” he was at a lose at this point he may need new eyes. =^= ” Computer, given the patients current scans and results, and current rate of never degeneration what are the recommended treatment options. ” =^=

No treatment is recommended until Echtallin poisoning has been neutralized.=^=

Tealor’s lips shifted sideways as he chewed on his cheek. “Well that was not helpful, Ensign we have to figure out a way to slow this stuff down, does it follow any kind of pattern?” he said looking how the poison has progressed so far.

Chief Medical Officer U.S.S Wolverine“The more active the person is, the faster it spreads and the more intense its effect.” The end was horrible. “Eventually, it settles in the brain, taking over control the entire nervous system.” She didn’t have any experience with the antidote. “All the cases I read said as the antidote takes effect the Echtallin amasses in one area, as if trying to do the most intense damage before it can be stopped. One of the doctors, Bonpiele, thought injecting the antidote into the brainstem would help slow the progress, but his patient died before he could try.”

Dradvia, Medical OfficerTealor looked at her and then to Zoi. “That’s very risky, ” he said trying to think through other options. “We don’t have much time to make a call before things get much much worse, we could put him in a coma and hope that slows things down enough, or we can try the antidote in the brainstem. Maybe we do both?”

“Coma is risky. With the current damage… he might not wake up.” Induced coma was much different to sedation. Dradvia racked her brain. “What about medical nantes? Loading them with the antidote and allowing them to create a barrier to the affected areas. Have them send out neural impulse signals to attract the poison? They aren’t attached to Sisnil or his system. The poison would be stuck in the nanite.” In theory.

Tealor smiled, he enjoyed having Dradvia on his staff, she kept this up she would be promoted soon. “I agree on the coma is very high risk, ” he responded, “I am liking the Nanite idea, run a few simulations feel free to enlist help from engineering if you need. ” he said thinking through the idea.

Tealor looked at him confused, “If you insist, but I would like to know why?” he said looking at him with concern, he wanted to use telepathy but he did not want Zoi to exert him self. He gave a concerned look to Dradvia.

Chief Medical Officer U.S.S WolverineTessan could tell she wasn’t too thrilled with the idea of Engineering either. She knew Sisnil’s objection was either his mistrust of the crew and need for privacy or it was because of Stevens.

It was both for Sisnil. He distrusted everyone, but the fact Stevens had already threatened or suggested harm, made him an active threat. In no position to protect himself, Sisnil couldn’t let the man near anything that had anything to do with him. “I don’t trust them,” was all he said.

He looked at Dradvia he would tell she was aware of his distrust before he mentioned it. “Ensign please run some simulations and give Mr Zoi and I a moment.” he ordered.

Chief Medical Officer U.S.S WolverineDradvia actually hesitated. She had given Sisnil her word she wouldn’t leave him alone in Sickbay unless it was Morris. Her word meant everything to her. She couldn’t defy a direct order though. With his vision gone, she wouldn’t be in his line of sight. He could sense her though, right? Yes, that part of his brain wasn’t damaged. He would know how close she was.
Dradvia bit her lip. “Yes, Sir.” She sounded as torn as she felt and meek. She gently squeezed Sisnil’s hand. “I’ll just be at the computer.” It was just across from the side of his bed. She immediately got to work on the simulations Tessan asked for.

“Okay. It’s okay, Dee.” He said softly. He could sense where she was. He focused on that. Irrational depictions of Tessan doing something unthinkable flashed through his head.
Tealor watched here move to the console, he lowered his voice. “What is going on? I’ve heard rumors about some hmm poor interactions with an engineer and one of my medical staff.” he said very quietly. “Did someone threaten you?” he asked.

Chief Medical Officer U.S.S Wolverine“No,” Sisnil claimed. Normally he’d be capable of pulling off lying to a telepath. Right now he could not maintain that kind of mental fortitude. “I’m fine.”

“If someone is causing you problems or threatening you it needs to be dealt with.” he said knowing that Zoi was not telling the truth. Tealor was not going to make the man argue, he was too fragile and his feeble attempts to block tealor out would only make him worse, he interrupted. “We’ll talk more about this later, since you don’t want to be honest now. ” he said “And stop using your gifts or I’ll inhibit them, it’s killing you”

Chief Medical Officer U.S.S Wolverine“I can-” Sisnil started, but Tealor interrupted him. “Don’t inhibit them.” He said, almost pleading. The pitch of his voice was sure to grab Dee’s attention. Tessan could sense the fear of an all encompassing loneliness.

Dradvia was purposely not listening to their conversation which was hard given the small size of sickbay. The tone in Sisnil’s voice caught her ear and she turned to look at him. Something was wrong. If he got upset his body could go into fight or flight and then the Echtallin would spread faster.
He looked at the man with compassion, “Look if I wanted to invade your mind I would have done it already, I’m also keeping you mind shielded and have been this entire time” he noted.
“I know,” he said softly. But too many times, he’d seen people seem just fine until the moment of betrayal came. That was what made it hard to trust him.
“Please for your own sake, relax and stop using them, otherwise i might not have a choice. ” he reached out with his mind. ‘I give you my word someone will be with you at all times, Dravida won’t leave you nor do I want her too it’s better you have someone with you’. he then paused. ’ I will keep your mind safe until you can do so on your own. I give you my word on that as well’ he said “Now rest please and stop using your abilities before it kills you”

Chief Medical Officer U.S.S WolverineThere was a sincerity to his words that seemed to help Sisnil relax. He let out a tired breath. “I can’t see, it’s the only way I can feel safe.”

Tealor stayed connected, “Ensign, pause what your doing and watch Mr Zoi a moment” he asked.
“Yes Dr. Tessan.” She paused her simulation and stepped back over to the bed. She set her hand against Sisnil’s. She watched Tessan walk away, worried and uncertain what was going on.
He did not wait for her to respond and moved to the back of sickbay, he pulled out one of the mobile terminals gave it access to to sickbays database. Moments later he came back and rolled the computer next to Zoi’s bed. “Ensign you can work on the nanites from here, so Mr. Zoi has some company,” he said ” please let me know when you are done with the simulations.” he requested.
“Yes, Sir.” It sounded more like a question. She sat down on the stool that she had placed by the bed. She transferred her simulations over.
As he turned to leave ‘Trust, Mr Zoi, goes both ways, please don’t make me regret my decision.’ he thought as he worked is way back to his office.

Sisnil squeezed her hand gently. It seemed it was difficult for him not to use his telepathic abilities. “He asked me to trust him, Dee.” He said. “To not use my abilities because it’s accelerating the poison in my brain.”

She turned to look at him. “I know you can’t trust him but trust me. He’s a good doctor. He won’t harm you. I won’t let him, but I won’t need to do anything. I promise.” She rubbed her hand over his. “I know it’s instinct, but try to not use your abilities. I’m working as fast as I can.”
She was watching the simulations while they talked.

“I know,” he said softly, “I trust you, Dee.” He desperately wanted to see the screen. “Any results?”

“Ummm, not yet. I put a lot of factors in. I didn’t want to miss anything.” There were a lot of factors to consider. They didn’t have any Echtallin to experiment with, and there wasn’t a lot known about it.

In a majority of the simulations, the nanites would cause movement within the brain, which would cause the poison to break down the nerves within the brain more quickly. No matter how they did it, it seemed there was a slim chance it would help, and a much greater chance their presence would accelerate the poison.
Sisnil’s body jerked painfully and he groaned softly. “How much longer of this?”

“Another day before the antidote treatment is finished.” Then they would have to see how much damage there was.
With the results complete, Dradvia paged Dr. Tessan.

Tealor walked back in he was visibly tired, he moved to the bedside look at Zoi’s numbers. “You have news Ensign?” he asked

Chief Medical Officer U.S.S WolverineDradvia turned the console towards him and shook her head. The results weren’t good. “The simulations only show a 15% success rate at best. Average is 7%.”

Tealor for the first time showed concern on his face. “That is not goodnews, We’ll have to go with the antidote only then it’s too high a risk” he noted. “Do you think we should, keep him awake during the procedure take him under” he asked.

Tealor looked at him. “We can do the Antidote in the brain stem, or do what we are doing both have risks” he said and looked to Dradivia to see what she thinks. There were not good options just different bad ones.

Chief Medical Officer U.S.S Wolverine“If we put you to sleep it would just be the safe sedative we’ve been giving you.” Dradvia thought over it. “The antidote is working. Your last blood count was 56% to 44%. My concern with directly injecting it to the brainstem is that the Echtallin reacts aggressively to the antidote in large doses. That is why it’s incremental dosaging.”
“What,” she paused thinking. “What if we put 1/4th if the dose in the internal carotid arteries and another fourth in the vertebral arteries? That way it is filtered and dispersed more naturally than into the brain fluid. We put the last half of the dose into the jugular vein, that way it spreads through the rest of his system to attack any Echtallin not in his brain.” It would also allow the antidote to be continually dispersed where needed over time.

Dradvia was a good doctor she he was always impressed by her ability to push through noise, even under high pressure. He thought for a long moment. “I like the idea so far” he noted a hint of doubt in his mind. “If we risk smaller more sensitive never clusters? I suppose anything other than the brain or the heart is a better risk, and we can replace the heart and many of the other system of the body,” he noted. “The brain on the other hand”
“Will it destroy my heart if you do that?” Sisnil asked, alarmed.
“I don’t think so. The Echtallin has moved away from your heart. There is damage, but it’s repairable right now,” Dradvia told him.
Tealor looked at Zoi, “You are healing, we just don’t know if it will be fast enough for there not to be irreversible damage to your brain. Ensign Dradvia plan is sound it separates the antidote in equal amounts around your body so the poison can not attach to one area.” he looked at her, “In tactical terms we are making the Echtallin fight a war on multiple fronts.”

She set her hand on his. “I think we should give you the sedative so your body can relax as much as possible, and then let us try giving you the antidote in three different locations.” She didn’t allow doubt or worry to enter her thoughts. She was confident and sure.
