- He’d suffered a deep wound to the upper abdomen that had been sutured.
- Hypovolemic shock due to blood loss
- Given two units of Betazoid type Z blood along with plasma during surgery
- Kayolane 175 ml for sedation and pain had no effect
- Drechtal Beams were used to cut nerve signals and prevent pain sensation
- Blood worked showed echtallin poisoning. It was made to cause nerve damage and hyperactivity, fever but no infection… the body created a crusty residue at the wound from the immune system the nerves, it increased pain, prevented loss of consciousness. It interacted with typical medications, making symptoms worse rather than relieve them. The poison itself was like nervous system acid, damaging and burning the nerves, causing central nervous system dysfunction, which explained his body’s temperature fluctuations. There was the potential for long term nerve damage and organ damage both from the poison itself and the effects of the central nervous system dysfunction. It would cause pain too. It was intended to be a slow, agonizing death as the poison slowly dissolved the nerves and the central nervous system slowly shut down.
Treatment for Echtallin Poisoning:
- hydrolyze treatment - It was a compound that would neutralize the poison, but do it too quickly and it would react with the poison instead and kill him. It had to be given in multiple doses per day for several days.
- Pain medication that blocked pain signals should work, but it had to block the pain signals, because the pain was directly from the nerves. Side effects of the treatment were liver or kidney toxicity, heart problems, numbness, and a whole list of other things. The dosing information was there, describing how to decide how much to give, when, and how.
- and only a specific type of sedative would be able to put him to sleep, because the way it acted to put him to sleep was different than most others.
- Even with the antidote treatment - survival was only 20%
- Given Bromocriptine to help regulate body temperature and hormone functions.

It was during the time after Sisnil had been attacked at the Starbase. He was still in Sickbay, stabbed in the abdomen and poisoned with an awful, rare, and largely unknown substance called echtallin. Dee had told Tessan all about it.
It was awful because the way it effected the body made most pain killers and sedatives ineffective. There was one of each that would work. At that time though, Sisnil was awake. He looked awful. His K3 indicator showed he was in quite a lot of pain. Dee had refused to leave his side, and they were talking. Mostly she was telling him to stay calm and rest.

Tealor walked in with his Padd and moved over to the bed to check the readings, “How are you holding up Mr Zoi?” he asked since he knew he was not well. “Dradvia could you excuse us for a moment please, go grab a snack or strech your legs.” he asked

Chief Medical Officer U.S.S WolverineDradvia blinked not wanting to leave, but an order was an order. “Yes, Sir. Of course.” She looked at Sisnil. “I’ll go check on Simon for you.” He wasn’t due for more meds for awhile, and the alarm would go off to alert Tessan when it was time.

Sisnil reached for Dee, not wanting her to leave. But she was gone. It was one of those rare times when Sisnil was too weak and unwell to maintain his mental walls, and Tessan could sense his uncertainty, unwellness, distrust, and paranoia. He glanced at Tessan, “You shouldn’t be here. You’re Betazoid. You can sense my pain.” He said, his voice weak. There was sincere care in the words, he wasn’t just saying it because he didn’t want to talk to Tessan, he genuinely didn’t want the man to feel what he was.

Tealor was not interested in searching through the mans head without his permission, and while he felt some discomfort he was able to manage it. “I can manage for now,” he noted. “If it becomes to much please don’t be offended if I have to leave” he added.
“I would protect you, I’m capable of that, but not right now.” Sisnil said. “There is no reason for you to put yourself through this. Dee is perfectly capable.”
He raised his hand, “She can participate, but given her level of, ” he thought about a safe word. “Concern for you, I’m taking over as primary, It puts the responsibility on me should something happen and not her” he stated in hopes he would catches his meaning, there was a good chance she was outside the door listening. “I’ll be fine” he said though Sisnil could see the discomfort on his face, he was indeed functioning ok.
“Is it ok with you that Ensign Dradvia hears your medical information, it was not authorized on your admission” he stated.
“She’s my doctor, she already knows it.” Sisnil said. “For purpose of your paperwork, yes, she can.”
Tealor looked over the stats and his current fluid runs. “I’m sorry I can’t do more for you. ” he apologized. “I could induce a coma but I’m worried that would trap you in a nightmare of pain that you could not wake up from”.
Sisnil shook his head vigorously. “I don’t want that.”
Tealor looked at his pain med regiment. “I’m looking at your current readings, I can’t increase the does on either of your pain meds. ” he noted.
“I know,” he sighed.
“I don’t see it in the notes, but back home, pressure therapy is used to control the sensations that your feeling” he paused, “Since the damage is internal I’m not sure it would help but it’s worth a try. It should not make it worse but it may just not do anything”

Chief Medical Officer U.S.S WolverineSisnil considered the idea. It could make it worse, he thought, if it caused his system to misfire. But the force field pressure thing Dee had used was helping. Sisnil eyed the man suspiciously like he might use the opportunity to do something unsavoury. But the idea of some relief of his pain was tempting. “…maybe.” he said at last.

“Let me explain it more, Its similar to the bio field that you are under now, but we’ll surround your entire body with a force field, it’s similar to barometric pressure treatment in that way.” he said, using the biomed sensors we target puting the same amount of pressure at every nerve ending. since every nerve ending is different it will take time for the sensors to register and adjust. Eventually you’ll be feeling the same thing everywhere, no changes from air passing over, blankets shifting you’ll feel no changes until the field is lifted.” he paused for him to let it sink in.
“The calibration will, i’m afraid, be excruciating to a level that would be unbearable normally but given your condition you will feel to the fullest and it takes minutes not seconds” he said “Or you deal with the pain you are currently under until poison works out of your system.”
He tapped a message to Dradvia in his Padd, “You can come in now” expecting that she was already waiting at the door, they would have to talk about the intensity of this relationship once this event was over.”

Chief Medical Officer U.S.S WolverineDradvia did not return right away. Even though she knew Sisnil didn’t trust Tessan, she knew Sisnil was safe with him. She had been given an order to take a break. So she did what she said she would do. Dradvia went and checked on Simon. She was not expecting a return summons so quickly. So she had to hurry to feed and clean up after the kitten and then down her snack on the way back to sickbay. She returned several minutes later. She moved over to the biobed, standing so that Sisnil could see both of them without strain. “Dr. Tessan?”

“Ensign, I was just going over some possible options for the pain, but there really are not any good ones” he said sending the details to her Padd,he looked at her a moment. “Given the complexity of this case, I’m taking over as primary physician, you are free to consult as long as you can maintain objectivity” he said to her plainly.

Chief Medical Officer U.S.S Wolverine“I’m willing to try it if it will make it hurt less,” Sinsil said. But he had objections to Tessan taking over. He shook his head. “Dee stays.”

Tealor raised his hand and was about to respond when Dradvia opened her mouth.
It was almost a blow to her confidence, but Tessan was more experienced and she wanted the best care for Sisnil. She looked over the information Tessan sent her while coming her thoughts. She turned so she could look at them both at the same time. “I do not recommend the pressure therapy. In the end it will lessen the pain, but over stimulation makes the Echtallin work faster. It will spread to his brain faster. Putting him under the strain of calibration will lower his chances of survival.”
She looked at Tessan, in the eye, her confidence and certainty showing through. “I am compromised, Dr. Tessan. I won’t deny it, but not because Mr. Zoi is my friend. Because I have seen what Echtallin does. I’ve sat beside people and eased them through to death in the midst of the torture. I couldn’t save them, I could only comfort them. No one, no matter who they are, should suffer from this stuff. I’m the only one on staff who has any experience with it. Yes, I am compromised because I don’t want to see anyone else die from this. But that will not stop me from doing what is best for the patient.” The two telepaths could easily sense her honesty, her determination, and her professionalism as she spoke.

Sisnil closed his eyes as if focusing. The pain was bad. All he heard was “lessen the pain in the end. “Dee, please,” he said, in an almost begging voice.

Tealor loved that she was being assertive, it was critical that she stay engaged and stand up for the patient and she was being good doctor, “Couple of things, ” he started. “One I’m not removing you from the case, as you say without your experience with the poison, it is unlikely that he would even be alive now to have this conversation.” it was clear that this was far more than your run of the mill friends ship, how much more would have to be determined at a later date. “Two, I’m taking primary, Ensign, it’s not up for debate. That said I expect you to continue performing your duties. ” he said, if something goes wrong or a decision was made that resulted in Zoi’s death, then she could hate me instead of herself for it he thought. “I expect to be informed before changing the plan, if there is an emergency I expect to be notified instantly” he stated firmly.
“I had considered that,I felt like enough of it had been contained that it would not react based on his last lab? Mr Zoi, I would prefer we not kill you let let her Explain”
