Lt Kelleher,
I am formally placing Ensign Serinde on report for recent actions and behavior that reflect poorly on her commission and on Starfleet as a whole. Such dereliction cannot go unreported.

Per regulation standard C194 Paragraph 3, it states that Starfleet personnel, while on active duty, must have an open communications channel in the event of an emergency recall. Ensign Serinde had turned off her communications and was late to see the recall to duty the evening before the departure of the Sojourner and was subsequently late for a meeting muster. As well, the following day, she did not acknowledge receipt of communications by myself of which I sent several inquiring of her whereabouts and held up the entire engineering squad and hindered them from more hands on time prior to launch.

Per regulation standard A28 Paragraph 4, it states that officers and enlisted persons must obey orders by a superior. Ensign Serinde was ordered to undertake decommissioning of the Bonaventure, more specifically, following the regulation checklist to safely shut down the mechanical systems of the Bonaventure in advance of any Station crew assessing the ship for disassembly or refit. Ensign Serinde DID decommission the mechanical systems, however, her subsequent orders were to, nevertheless, muster on the Sojourner at 09:00. Serinde was late in coming and subsequently missed any chance of an orientation and familiarization tour of this new vessel. This is not a reflection on her character in intentionally obeying orders, per se, but shows her inexperience in a command role in the choices she made. Ultimately this is a good learning experience on her part which I hope she takes to heart.

Per regulation standard I311, it states that classified documentation must not be accessed when not authorized. Ensign Serinde attempted this not once but eight separate times on her late flight to the Sojourner. This was flagged by Station security and passed on to myself. You, Lt, must not have been fully in their system yet else you would have received this rather than I. This is a basic electronic security protocol that was in Ensign Serinde’s education. It should not have been missed.
Ensign Serinde’s conduct in this 24 hr period is inexcusable. Her one redeeming note was that she sent the Gamma shift on ahead to the Sojourner so they would not miss the 09:00 muster.

In light of all of this, I respectfully recommend:
A. That until Ensign Serinde has cleared remedial orientations and textual instruction and a high standard pass on the exam, she should not be allowed to operate or go near the Cochranes on the Sojourner. She is simply not qualified or familiar with this new equipment, though would have been should she have made it to the orientations. Until then Ensign Serinde should be limited to the Machine Shop.
B. Station Security has flagged her record that the infraction will not be cleared until she has taken the remedial basic security orientations as noted by the Station. There are three of them.
C. That for her dereliction and embarrassment to the Fleet Ensign Serinde should be assigned as Service Support** Liaison and be on call with Service Support for no less than a month, or greater as is in your purview.

** Service Support Liaison is, in essence, mechanically supporting the support personnel on board such as the cooks, the gardener, the cleaning staff, etc. If anything is required by them be it from a breakdown, an assist to something missing or broken, or significant clean up, the SSL would be there to assist them.
